Bilaterality of Bulla in Spontaneous Pneumothorax

Abstract

|
Forty one patients were operated for resection of bullae of lungs. Twenty one of them applied median sternotomy for simultaneous bilateral resection of bullae and the others underwent unilateral thoracotomy. Comparing the findings of chest PA and
chest
CT with the operative finding reveals that chest PA had poor sensitity for the detection of bulla, chest PA was effective for the detection with sensitivity of 92.3% and accuracy of 93%. Nineteen out of 21 patients had bilateral bullae(90.5%).
Three of
them had more serious bullous lesion in contralateral side.
In conclusion, the bullous lesions of the lung have tendency of bilaterality. In young patients with spontaneous pneumothorax who are going to have thoracotomy chest CT should be taken for the detection of hidden contralateral bulla for the
simultaneous
removal of bulla through median sternotomy to prevent relatively high incidence of contralateral pneumothorax.
